No walk-in window, even in the capital
It's tempting to think that living in the state capital means you can sort out a registry problem face to face. For the medical cannabis program, you can't. IDPH states plainly that there is no onsite assistance. The program works through:
- Email: [email protected] (OAPP: [email protected])
- Phone: 1-855-636-3688, Monday to Friday, 9 a.m. to 4 p.m.
- The online patient system, with a State of Illinois ILogin account
If you want a person across a table, IDPH points to county and local health departments, which help with applications, renewals and extensions for free. In Sangamon County, start with your local health department. The application guide shows what you'll need before you go.
A year of changes, in order
2026 has been a busy year for Illinois cannabis law. If your information predates it, here's what moved:
| Date | Change | Status of the source |
|---|---|---|
| June 12, 2026 | SB 3222 signed: adult-use limits doubled, patients may buy seeds, curbside and drive-through allowed with IDFPR approval | Official (IDFPR fact sheet) |
| June 12, 2026 | In-person exam no longer required for certifications | Secondary (news report of an IDPH email); confirm with IDPH |
| July 1, 2026 | 7% cultivation privilege tax on cultivators' first sale of medical cannabis | Official (IDOR); paid by cultivators, not at the register |
| September 10, 2026 | Adult-use dispensaries may opt in to medical licenses; patient allotment purchases there exempt from purchaser excise and local cannabis taxes | Official (IDFPR, IDOR) |
| November 12, 2026 | Products with more than 0.4 mg total THC per container become "cannabis," sold only under the adult-use law | Official (IDOR) |
The full rundown, verified items first, is in our SB 3222 explainer. The in-person exam question has its own page: online evaluation in Illinois.
Need the certification first?
IDPH won't open your application until an Illinois practitioner certifies you. Book an online evaluation with a licensed practitioner; code DRWDY85872 takes 5% off. The practitioner, not the booking site, decides whether you qualify.

The September opt-in matters most locally. Any adult-use dispensary in good standing can now apply for a medical license, so the number of Springfield-area stores where your card lowers the tax may grow. Check the state's map at cannabis.illinois.gov/about/locations.html and filter for medical sales. We don't name dispensaries.
At a medical-licensed store, a patient buying within the allotment skips the 10% excise on flower at or below 35% THC, pays the 1% qualifying-drug state rate instead of 6.25%, and pays no local cannabis tax.
