Cutting the state fee
A Rockford resident pays the Illinois Department of Public Health the same registry fee as anyone in the state, but the choices inside that fee matter. Here's what each option costs per month of coverage:
| Option | Fee | Per month |
|---|---|---|
| 1-year card | $50 | $4.17 |
| 2-year card | $100 | $4.17 |
| 3-year card | $125 | $3.47 |
| Reduced fee, 3 years | $75 | $2.08 |
| OAPP, 90 days | $10 | $3.33 |
The two-year card is the one with no discount at all. If you plan to stay on the program, the three-year card is cheapest. The full fee breakdown includes caregiver bundles.
Who pays less
Veterans with an honorable or general discharge who are eligible for VA services pay the reduced fee: $25, $50 or $75. They need a DD-214, and those treated at a VA facility can submit 12 months of VA records instead of a certification, which removes the evaluation cost entirely. The veterans page explains both.
Certain other groups may qualify for the reduced fee; IDPH lists them, so check before you pay the full rate. And under the Act, terminally ill applicants pay nothing.
OAPP as a cheaper trial
The Opioid Alternative Patient Program costs $10 per 90 days, for Illinois residents 21 and older with a condition for which an opioid has been or could be prescribed. A physician must certify it. Four periods cost $40 a year, $10 less than a one-year card, at the price of renewing every quarter. It's a different program with different rules; see the OAPP guide before choosing it.
Need the certification first?
IDPH won't open your application until an Illinois practitioner certifies you. Book an online evaluation with a licensed practitioner; code DRWDY85872 takes 5% off. The practitioner, not the booking site, decides whether you qualify.

IDPH says county and local health departments help with applications, renewals and extensions, and "there is no charge for this assistance." You're still responsible for the fee and a passport-style photo. IDPH has no walk-in help of its own, so if you'd rather sit down with someone, start with your local health department in Winnebago County.
Where the savings come back
The fee is only half the math. At a dispensary with a medical license, from September 10, 2026, a patient's purchases within the allotment skip the purchaser excise and local cannabis taxes. On flower at or below 35% THC, that's at least 15.25% of the price. To find medical-licensed stores, use the state's map at cannabis.illinois.gov/about/locations.html. We don't list individual dispensaries.
